Creator Edmund McMillen told an AMA on the Steam Deck subreddit that "It will take an average player about 200+ hours to 'beat the game' and 500+ hours to 100%". At the time of writing I have about 115 hours logged and the save screen shows 83% completion, with the main questline still not fully finished.

Part of that estimate could come from the game's new-game-plus–style options, which let you replay levels and bosses on a harder difficulty to unlock more content. If clearing those runs is counted as part of "beating" the game, 200+ hours becomes more plausible.

Progress will vary; I played intensely for roughly three weeks, which likely sped my learning curve, while longer breaks between runs could mean more failed attempts. There are also many hidden achievements and ultra-rare "cryptid cats"—shinylike special cats that have so far been spotted only once among reviewers and creators—which could extend the time needed to reach 100%.
