People reports Samantha Milton, known as "Smaller Sam PCOS" on social media and a cover star of its "How I Lost 100 Lbs." issue, has lost 260 lbs after reaching 400 lbs at her heaviest. Milton, 29, said she was diagnosed with polycystic ovarian syndrome at 12 and struggled with disordered eating and weight cycling; after her first daughter she developed postpartum anxiety and depression and gained about 200 lbs over five years while coping with her husband's alcoholism.

With limited time and money, she focused on maintaining a calorie deficit, exercising and adapting meals on long car trips with lower-calorie, higher-protein fast-food options — including a Chick-fil-A kale salad with chicken strips and honey hot sauce that has 8 million views on TikTok.

Now a full-time influencer in Ashburn, Va., Milton has more than 4 million followers and endorsement deals. She maintains a 140-lb. weight by eating fewer calories than she burns most days, treadmill workouts and taking GLP-1 drugs, which she began two years ago and now microdoses at a monthly cost of $299; she says they help treat her PCOS symptoms and quiet her mind and food noise.
