While JD contends with a friend who is too busy to spend time with him, the other doctors at Sacred Heart handle a case that reflects modern eating and fitness trends.

Guest star Matt Rife plays Logan Nichols, a fitness influencer following the Tarzan diet, which largely avoids carbs and fruit in favor of protein. The lack of fruit causes bleeding gums; Elliot diagnoses scurvy and prescribes fruit, but Logan repeatedly refuses.

Newcomer Dr. Sam Tosh (Ava Bunn) combs his social media and suspects orthorexia nervosa — an intense fixation on eating what one perceives as healthiest — noting his refusal to eat fruit and his laser-focused weight-loss efforts even when they harm his health. Elliot and Sam agree to help him through it.
