Movieweb reports that 28 Years Later's two-part sequel concludes with The Bone Temple, which reveals a cure for the rage virus and positions the story for a final installment.

Dr. Ian Kelson elects to study rather than kill the infected Alpha, Samson, using morphine-infused darts and medicine for psychosis to show the virus covers rather than replaces consciousness. Samson regains awareness at the train where the original outbreak occurred, retains his memories, size, strength and an apparent immunity, fights back to the Bone Temple and finds the dying Kelson, giving the doctor a measure of closure.

The film also returns Cillian Murphy's Jim, who remains in the house from the first film and now has a daughter, while Selena's absence raises questions. After Kelson's confrontation with Sir Jimmy Crystal leaves most characters dead or fatally wounded, Spike and Kelly survive and later join Jim and his daughter; Sir Jimmy is crucified upside down and dies. Several threads remain open, including the pregnant woman Cathy, Jamie and the island community tied to Spike.

The piece draws biblical parallels and frames the ending as the start of a potential rebirth for humanity; in 2024 it was revealed that Alex Garland will write the final installment with Danny Boyle returning to direct, indicating the saga will continue and leave remaining questions to be resolved.
